Step 1: Download the Application

Step 2: Gather Required Information
You'll need to have the following details ready:
Office Information

Firm Name and Office Name (Branch name should differ from main office)

Branch Office Address (Must be a physical location—no P.O. Boxes)

Phone, Fax, and Email for the office
Managing Broker Info

Name of Managing Broker for the new branch

Broker License Number

Step 3: Indicate the Type of Application
